Ujjwala Dheeriya, MD
Senior Medical Officer for Palliative Care, Inland Empire Regional Medical Director
Dr. Dheeriya has been practicing medicine since 2000, specializing in the care of frail, elderly and vulnerable populations. She is double board-certified in Internal Medicine and Palliative Care. She is a graduate of University of Virginia School of Medicine and completed residencies in Internal Medicine and Pediatrics. Her experience includes primary care, hospital medicine, and facility-based care. She raises service dogs and enjoys cooking, listening to audio books, attending concerts with her husband, and spending time with her two teenage sons. She is often spotted poolside watching them play waterpolo. Dr. Dheeriya is dedicated to optimizing quality of life for patients through building compassionate relationships with them and their families.
Orlando Collado Jr., MD
Orange County Regional Medical Director
Dr. Collado has been practicing with CareConnectMD (formerly Gerinet Medical Associates) since 2009. He is a graduate of De La Salle University College of Medicine. He trained at the University of Tennessee Memphis and is board-certified in Family Medicine. He specializes in the care of post-acute and rehab patients. His interests also include integrative and preventative medicine. When he’s not seeing patients, he enjoys spending time with family, surfing, practicing yoga, being in nature, golfing, basketball, shopping at farmers markets, and traveling. His most memorable patient moment would be providing hospice care in 2017 to his father during his final weeks at home.
Simon Chang, MD
Los Angeles Regional Medical Director
Dr. Chang specializes in the care of post-acute and rehabilitation patients, as well as geriatrics. He has been practicing medicine since 2001. He is a graduate of Northwestern University Feinberg School of Medicine. Prior to CareConnectMD, he worked at CareMore Health System and in private practice. His interests also include making house calls in the comfort of his patients own environment and teaching. When he is not seeing patients, he enjoys hiking and traveling.
Flora Danque, MD
San Diego Regional Medical Director
Dr. Danque is a family physician and the San Diego County Regional Medical Director at CareConnectMD. She is a graduate of UC Irvine School of Medicine and has been practicing since 1997. Prior to CareConnectMD, she worked in private practice, as well as served as the medical director of a hospice agency and several skilled nursing facilities. She specializes in family medicine, geriatric medicine, and hospice and palliative care. When she is not seeing patients, she enjoys spending time with her two sons, leading a healthy lifestyle, doing martial arts, and volunteering with AIDSWALK San Diego as Medical Director. Dr. Danque believes if you allow your patient to talk long enough and truly listen to their concerns, without interruption, they will usually tell you what’s wrong.
