Chief Medical Officer
Dr. Liliane Idylle serves as Chief Medical Officer of CareConnectMD Inc. (CCMD) where she oversees clinical strategy, provider education, and leads the organizations top performing value-based care program. An accomplished expert in Internal Medicine, Dr. Idylle brings extensive experience in the care of elderly, vulnerable, and medically complex patient populations. She leads CCMD’s value-based care initiatives and fosters strategic partnerships with partner provider groups—ensuring alignment around patient outcomes, care efficiency, and system-wide quality improvement. Prior to joining CCMD, Dr. Idylle served for a decade as Medical Officer at CareMore Health Plan (Anthem). Dr. Idylle holds a Doctor of Osteopathy from Des Moines University College of Osteopathic Medicine and completed her Internal Medicine residency at LAC and USC Medical Center. She earned her Bachelor of Science degree in Biochemistry from the University of California, Los Angeles (UCLA).
Adam Solomon, MD, MMM, FACP
Senior Medical Officer
Dr. Solomon is the Senior Medical Officer overseeing the post-acute line of business across Los Angeles, Orange, and San Diego counties. He brings extensive experience in clinical leadership, population health, and value-based care. Prior to this role, he was Chief Medical Officer at Memorial Care Medical Foundation, where he led initiatives in disease management, patient experience, and virtual care across more than 200 sites. His background also includes leadership roles at Regal Medical Group and the Physician’s Choice Foundation. Dr. Solomon earned his MD from the University of Maryland, completed residency at the University of Utah, and holds a master’s in medical management from USC.
Orlando Collado Jr., MD
Regional Medical Director
Dr. Collado is the Regional Medical Director at CareConnectMD and has been with the company since 2009. He leads clinical teams in post-acute and long-term care settings, ensuring quality, compliance, and improved outcomes for high-needs patients. Board-certified in family medicine and integrative medicine. Dr. Collado earned his M.D. from De La Salle University in the Philippines, completed residency at the University of Tennessee’s St. Francis Family Medicine and holds a B.S. in Biological Science from UC Santa Barbara. Multilingual in English, Tagalog, and Spanish, he is an active member of the American Academy of Family Physicians.
Kerry Weiner, MD
MSSP Medical Director
Dr. Weiner is the Medical Director for the Medicare Shared Savings Program (MSSP) at CareConnectMD, bringing over 45 years of clinical and leadership experience in internal medicine, gastroenterology, and value-based care. A graduate of the David Geffen School of Medicine at UCLA, where he also earned his Master of Public Health, Dr. Weiner has held senior roles including Chief Medical Officer at IPC-Team Health and Lakeside Community Healthcare. Board certified in Internal Medicine and Gastroenterology, he has played a pivotal role in developing innovative care models for frail and medically complex populations across skilled nursing, post-acute, and home-based settings. His leadership in the MSSP reflects a deep commitment to aligning clinical outcomes with accountable care performance, quality metrics, and regulatory compliance, while mentoring clinical teams and collaborating with federal agencies like CMS and CMMI to advance patient-centered delivery systems.
Wanda Colon Cartagena, MD
ACO Medical Director
Dr. Colon-Cartagena is the Medical Director for the Accountable Care Organization (ACO) at CareConnectMD, where she leads clinical strategy and operations to improve outcomes for seniors and patients with chronic conditions. A board-certified physician in Internal Medicine and Geriatric Medicine, she has over a decade of leadership experience in value-based care, including serving as Market Medical Director at Landmark Health/Optum. Dr. Colón-Cartagena is recognized for her expertise in geriatrics, palliative care, and interdisciplinary team development, and is committed to advancing innovative, patient-centered models that enhance quality, coordination, and compassion in healthcare. She earned her Doctor of Medicine and Master of Public Health in Epidemiology from the University of Puerto Rico and completed her residency and fellowship training in Internal Medicine and Geriatric Medicine at Baystate Medical Center/Tufts University School of Medicine.
