Our personalized approach ensures you receive the right level of care and resources to effectively manage your health and well-being. Whether you need care coordination, caregiver support, or access to specialized services, we'll design a plan that's tailored to your unique situation.
-
If you're a family member or unpaid caregiver providing support to a loved one, the GUIDE program offers education and resources to help you in your caregiving role.
This includes:
Direct communication with a dedicated care navigator
Educational materials and training on caring for someone with dementia
Connection to support groups and respite services to give you a much-needed break
We understand the demands and challenges of being a caregiver. That's why our program is designed to provide you with the tools, information, and personalized assistance you need - whenever you need it.
-
The GUIDE program offers respite services to give family caregivers a much-needed break, up to an annual limit. This allows you to recharge and refresh, so you can continue providing the best possible care for your loved one.
Respite support is available through:
In-home respite providers who can temporarily take over caregiving duties
Adult day centers that provide engaging activities and supervision
Short-term stays at nursing homes or other care facilities
No matter which respite option works best for your situation, we'll connect you with the right local providers to give you the time and space you need. Your wellbeing is just as important as the person you're caring for.
-
As part of the GUIDE program, we'll connect you with a range of community-based services to help meet your needs, including:
Meal delivery
Transportation assistance
Home safety modifications
Social activities and engagement
But that's not all. Our dedicated care teams will work closely together to coordinate all of your clinical and support services, ensuring you receive seamless, comprehensive care.
Whether you require medical treatment, in-home help, social support, or a combination of services, we'll manage the details so you can focus on your health and wellbeing.
By accessing this full spectrum of resources through the GUIDE program, you can continue living independently in your community for as long as possible.
Let us connect you to the services and support you need. Get started today.
-
As part of the GUIDE program, you'll be assigned a personal care navigator to help you every step of the way. Your care navigator will:
Assist you in accessing the care and services you need
Coordinate your medical treatment, in-home support, and community resources
Provide personalized guidance and answer any questions you may have
But the support doesn't stop there. GUIDE also offers 24/7 access to a member of your care team, whether that's through a dedicated helpline or direct communication.
Whenever you need advice, have a concern, or simply require a listening ear, your care team is just a phone call away. You'll never have to face the challenges of dementia alone.
Let your GUIDE care navigator be your constant source of support. Contact us to get connected.
