Clinical Support

Enhance your ability to provide comprehensive and coordinated patient care when you partner with CareConnectMD as your ACO. You receive resources, tools, and support, including data analytics, care management support, and care coordination services.

When you have 6 beneficiaries in your care, CareConnectMD will assign a dedicated case manager to be on-site for ½ days a week. The case manager’s hours increase based on the density at your site. In addition, CareConnectMD subsidizes Third Eye after-hours/weekend on-call services so your beneficiaries will always have the care they need, no matter when they perceive an issue, reducing the need to transport them to an ER.

 
 

In 2023, SNFs working with CareConnectMD experienced a significant decline in readmission rates, from 28% to 18%.
