CareConnectMD at Home DCE is 1 of only 8 high needs programs in the country to be selected for partnership with Medicare in this exciting new forward thinking model of care.
After over 25 years of delivering clinical results for the fragile and seriously ill populations CareConnectMD launched CareConnectMD at Home in April 2021. In partnership with Medicare, we deliver wrap around quality care to high needs patients wherever they call home.
CareConnectMD at Home is a team of doctors, hospitals, behavioral specialists, social health professionals and other healthcare providers who agree to work together aligning care, and enabling you to live your best quality of life, whatever your individual state of health. We work to coordinate your individual care, protecting your medical records and privacy, preventing preventable health crisis, avoiding unneeded Emergency Room trips, reducing duplicate tests and complicated paperwork costing you time and money. Allowing you and your family to be actively involved in your healthcare decisions. Lifting healthcare burdens to improve everyone’s well-being.
WHO IS ELIGIBLE?
A patient is eligible to be part of this service if they have Medicare or Medi-Medi, and meet the criteria deemed as high-needs.
WHO PROVIDES THE CARE?
Your physician who is participating in our high needs direct contracting program. Our CareConnectMD at Home nurse practitioners will also collaborate with your physician to provide more care.
WHERE IS THE CARE PROVIDED?
We provide care wherever you call home, whether that is your residence, a skilled nursing facility, or assisted living facility.
HOW WILL CARECONNECTMD AT HOME BENEFIT ME?
Your primary care physician has chosen to participate in this CareConnectMD at Home because they have access to additional resources that will allow them to provide quality care. You do not have to change your primary care physician nor will your Medicare benefits change. You may still go to any doctor, hospital, or other healthcare providers that accepts Medicare. However, because your doctor is now connected with CareConnectMD at Home, some special features may be available to you at no extra cost. These special features are listed below:

In-home wellness programs & assessments.
Use of telehealth and remote monitoring.
Behavioral health coordination.
Education and caregiver support.
Assistance in completing an Advanced Directive/POLST.
Dedicated care coordinator and social worker.
Palliative and end-of-life care.

HOW CAN I GET STARTED?
If you are a Medicare patient, please talk with your primary care physician to find out whether they are a participant in the CCMD at Home Program. If they are, please complete and return the Voluntary Alignment form to confirm that CareConnectMD at Home is your main doctor or healthcare professional for routine primary care.
